How to Get Working Papers
Guidelines for Working Paper Applications
1. Download the application from the link below:

Fill it out through ADOBE Acrobat - Complete Part 1 Only

Print the form
-
Have your parent or guardian sign the application
2. Physician's clearance stating that you are eligible to work (Doctor's Note)
3. Proof of age (copy of ONE (1) of the following documents: birth certificate, passport, non-driver ID or driver's license)
Scan all documents, save a copy for your files, and email all documents with your name as the file - Example Johndoe.pdf

Submit the documents to Ms. Craig, the Parent Coordinator in Room 129 or Email them to CCraig2@schools.nyc.gov
